I hope the ballroom is stopped and the next president builds an outhouse on that site with all his memorabilia dropped in a two holer with his face painted on the bottom.

It’s unlikely he’s aced it four times. Why would they do it so often otherwise? Strike option 1.
I don’t imagine he’d talk about it and know there are 30 questions if he hadn’t taken it. Strike option 2
He’s more than comfortable with lying about anything that reflects poorly on him. Or perhaps he’s incapable of processing negative feedback and the falsehoods are less a conscious lie than a defense mechanism indicative of mental illness. I’d buy that, but…
He gets angry and lashes out at any negative commentary so option three would be a tricky situation for clinicians telling him he’s suffering from mental infirmities, especially if they wanted to remain employed. Option 3 is plausible but not a clear front runner.
Which leaves option 4. There are a lot of people cashing in on their proximity to him - either direct financial benefit or the advancement of their own political ambitions - and that largely ends the day he stops being potus. It would be a serious ethical failure for physicians to not be honest with him about test results, but with pressure from family and cabinet level officials I could see someone agreeing to play along within some mutually agreed upon framework that if he declined to some level, they would take action. And it’s not like there aren’t other Ronny Jacksons out there who would make clearly false statements about the president’s health.

It is option 2. He took it in the past but he didn't take it this most recent visit. The fake medical report just added it because Trump likes bragging about his perfect score. There would be no diagnostic reason to administer it again.

Yep. The number of Americans who are only happy when millions of their fellow citizens are being made miserable is appalling, but it's also the reality we're all living with. It's like a woman who was interviewed in a 2017 article after Trump's first election said. She lived in a small Florida town whose economy was based around a local federal prison that employed a good many of the town's residents. Under Trump the prison was closed, and the lady (a Trump voter) complained to a reporter that Trump "wasn't hurting the right people." And that's still their attitude nearly a decade later - Dear Leader is their revenge on liberals for electing a black man president and getting gay marriage approved and all the rest.

His doctor is not an MD; He is a DO ( Doctor of Osteopathic Medicine )
He has been a career military physician and had an admirable record as an emergency doctor serving in the field during the Iraq and Afghanistan wars.

It looks like there is actually one competent person in the administration. Wonder if his work with people who had neurological disturbances after IEDs was a factor?

You're absolutely right from a good governance perspective. But Trump would VASTLY prefer to be reviled than forgotten. And my amygdala wants whatever would hurt Trump the most.
