He lives in a complete fantasy world of his own design, and likely has for his entire adult life. When you have that much money you can simply create your own reality and surround yourself with sycophants who will feed your fantasies and only tell you what you want to hear. In Trump 1.0 he did have some people around him who were willing to stand up to him and tell him "no" or just give him the truth. For his second term he's made certain that no such people were hired to assist him, and he's now surrounded by toadies like Patel and Rubio or clueless idiot braggarts like Hegseth or Noem or true believers like Miller who are willing to do his bidding down to the smallest and pettiest detail. Which is why, as awful as his first term was, he's much more dangerous this time.

OHSU says emergency flights will be diverted amid planned 'laser party' in South Portland

....A "laser party" for those frustrated with helicopters' nightly flights over the South Portland area can have dangerous consequences for patients, OHSU warned.

....Due to that planned event, OHSU said, several air ambulance vendors will not land at the hospital helipad Saturday night. Instead, their helicopters will land at other airports, and patients will then be transported to OHSU's hospital via ambulance, which could take 45-60 minutes

....."For most patients[,] that will be an acceptable delay. However, for some sensitive situations, such as unstable trauma patients, STEMIs and strokes, the delay could have real impacts," OHSU warned in an email.
